Job Summary:

We are committed to providing high-quality care to our residents and are seeking a skilled and caring LPN/LVN to join our team. As a Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) at Santa Clara Post Acute, you will have the opportunity to work with a diverse population of residents and provide individualized care to meet their unique needs.

Responsibilities:
  • Deliver high-quality nursing care to residents, including administering medications, conducting daily rounds, and monitoring resident care provided by unlicensed staff.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each resident.
  • Provide education and support to residents and their families on health-related topics and promote a culture of safety and wellness.
  • Maintain accurate and detailed records of resident care and communicate effectively with the healthcare team.
Requirements:
  • Current licensure as a Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) in the state of California.
  • Strong communication and organizational skills, with the ability to work effectively in a fast-paced environment.
  • Basic knowledge of computer use or willingness to learn.
Preferred Qualifications:
  • Experience working in a Skilled Nursing Facility (SNF) environment.
  • Nursing experience providing direct care to multiple residents.
  • Behavioral care / memory care experience.
  • Experience with Point Click Care (PCC).
