At Phoenix Home Care and Hospice, we are seeking an experienced Licensed Practical Nurse (LPN) to join our Home Health team in the Wichita, KS area. As a dedicated healthcare professional, you will be responsible for providing skilled nursing visits to our clients in their own homes.

Job Description

This is a unique opportunity to work in a home health setting, where you will be making regular visits to our clients to manage their healthcare needs. Your strong assessment skills and ability to communicate effectively with clients and families will make you an ideal candidate for this role.

The successful candidate will have a Kansas or compact nursing license, as well as experience working in home health. A valid driver's license and vehicle with current auto insurance are also required. Key Responsibilities
  • Provide skilled nursing visits to clients in their own homes.
  • Assist in the development, review, and revision of individualized care plans.
  • Monitor and report changes in client condition to appropriate personnel and/or physicians.
Compensation

We offer a competitive salary of $45,000 - $55,000 per year, depending on experience, plus an additional weekend wage. You will also receive paid mileage and a comprehensive benefits package, including multiple major medical plans, dental, vision, and life insurance.
