About the Role:

The Case Manager will provide essential support services to individuals and families in need, ensuring they receive the necessary resources and assistance to achieve stability and self-sufficiency. This is a full-time position that requires strong communication and problem-solving skills, as well as the ability to work effectively with diverse populations.

Schedule and Salary:

This role offers a competitive salary of $16.50-$18.25 per hour, based on qualifications, and a flexible schedule of Monday-Thursday 7 am - 5 pm or Monday-Friday 8 am - 4 pm.

Responsibilities:

  • Provide Support Services: Offer ongoing support to clients through home visits, phone calls, and written correspondence, with a goal attainment rate of 65% or above.
  • Assess Client Needs: Ensure clients are assessed through our internal process to determine possible assistance through available resources.
  • Develop Personal Plans: Create family personal plans to develop short and long-term attainable goals with families.
  • Maintain Confidentiality: Maintain a non-judgmental attitude toward clients and other CNCAP staff while ensuring client confidentiality.
  • Promote Programs: Promote the program and agency in a positive and professional manner.

Requirements:

  • Education: High school diploma or GED required; Bachelor's degree in human services or related field preferred.
  • Experience: 1-3 years of experience in human services or a related field.
  • Skills: Problem-solving skills, communication skills, knowledge of word processing, excel, PowerPoint, and databases, excellent time-management and organizational skills, and demonstrated ability to work with others.
