Key Responsibilities- Coordinate care and ensure continuous assessment of each patient's and family's needs
- Implement the interdisciplinary care plan in accordance with physician's orders and established plan of care
- Ensure delivery of care is compliant with federal and state regulations
- Graduate of an approved school of nursing education
- Licensed in the state where they currently practice
- At least one year experience as a licensed registered nurse (RN)
- Basic demonstrated technology skills, including operation of a mobile device
- Medicare home health or hospice experience
- Valid state driver license
- Automobile liability insurance as required by law
- Dependable transportation in good working condition
- CPR certification for the healthcare provider
- $5,000 Sign On Bonus
- 30 days PDO - Up to 6 weeks (PDO includes company observed holidays)
