About the Role

We are seeking a highly skilled Registered Nurse to join our Care Team at CHAS Health. As a key member of our team, you will play a vital role in improving the overall health of our communities by managing patient care at our assigned clinic.

Key Responsibilities
  • Perform independent visits while operating under standing order procedures.
  • Partner with providers on chronic care management and complex patient management.
  • Perform medical flip visits with providers for hospital discharges, ER follow-ups, and other complicated visits.
  • Facilitate the general day-to-day flow of the clinic in close collaboration with other team members.
  • Serve as a liaison between outside providers/healthcare entities and the clinic.
  • Collaborate with appropriate teams to design and test workflows and RN integration into the care team.
  • Perform patient, family, and caregiver education, including motivational interviewing and patient-centered health action plans.
  • Assist in improving the performance of identified quality measures and population health initiatives.
  • Triage patients in clinic and over the phone as requested.
  • For staff assigned to a specialty support role:
    • Responsible for case management related to specific patient populations to ensure utilization of specialized healthcare services.
    • Conduct regular assessments of patient needs and develop comprehensive plans of care.
    • Collaborate with patients and the Care Team for wrap-around services.
    • Conduct home health visits as determined in the care plan.
Requirements
  • Associate degree in nursing required.
  • Valid RN license in the state of Washington or Idaho required upon hire.
  • Nursing experience, preferably within a community health setting, required.
  • Case management experience preferred.
  • Experience with electronic health records preferred.
