Key Responsibilities
  • Financial Navigation: Conduct financial screenings for Ryan White, ADAP, and ADAP co-pay services, ensuring compliance with organizational and Division of Public Health policies.
  • Care Coordination: Collaborate with community case managers and agencies to provide a comprehensive continuum of patient care and maximize utilization of available resources.
  • Psychosocial Support: Evaluate and provide psychosocial support to patients and their families/significant others, addressing personal and environmental difficulties that predispose illness or interfere with medical care.
  • Resource Connection: Assist patients in accessing appropriate supportive services in the community, including Ryan White and ADAP funding.
  • Education and Outreach: Act as a resource person and assist in educational efforts involving HIV, with particular emphasis on psychosocial factors.
Requirements
  • Bachelor's degree in social work or related field from an accredited college/university required.
  • Master's degree in social work (MSW) from an accredited graduate school of Social Work preferred.
  • Licensed BSW or MSW as required by the state of Delaware.
