Job Title: Ambulatory Care Pharmacist
At Disability Solutions, we're seeking a highly skilled and experienced Ambulatory Care Pharmacist to join our team. As a key member of our pharmacy team, you will collaborate with physicians, interdisciplinary team members, and pharmacy leadership to deliver pharmaceutical care for eligible patients in the ambulatory setting.
Key Responsibilities:
- Collaborate with healthcare professionals to develop and implement individualized patient care plans.
- Conduct medication therapy reviews and provide education to patients and caregivers.
- Participate in quality improvement initiatives and contribute to the development of pharmacy policies and procedures.
Requirements:
- PharmD or Bachelor's degree in Pharmacy required.
- Completion of an American Society of Health System Pharmacy PGY 1 residency or 3 years of applicable experience required.
- Certification by the Board of Pharmaceutical Specialties in Pharmacotherapy or Ambulatory Care preferred.
Compensation and Benefits:
We offer a competitive salary range of $110,000 - $140,000 per year, depending on experience, as well as a comprehensive benefits package including health insurance, retirement savings plan, and paid time off.
