Position Overview

We are seeking a dedicated RN Case Manager specializing in Hospice Care. This role is designed for a Per Diem professional conducting Home Visits within the Hospice framework. Previous experience in Home Health or Hospice is essential.

In your role as the RN Case Manager, you will take full nursing responsibility for implementing the Plan of Care for each hospice patient. You will continuously assess both personal and professional performance, making necessary adjustments to enhance productivity and the quality of care provided.

  • Conduct initial nursing evaluations to determine eligibility for hospice services within forty-eight (48) hours of referral.
  • Assess the physical, psychosocial, and environmental needs of patients and families, re-evaluating at least every fifteen (15) days.
  • Initiate and coordinate the care plan while maintaining precise and relevant clinical documentation regarding the patient's condition.
  • Document issues, appropriate objectives, interventions, and responses from patients and families regarding hospice care.
  • Collaborate daily with patients, families, attending physicians, and other interdisciplinary team members to ensure comprehensive care.

Experience Requirements

  • A minimum of one year of clinical experience is required.

License Requirements

  • Current RN licensure in the state of practice along with one year of clinical experience.
  • Current CPR Certification.
  • A valid driver's license, vehicle insurance, and access to reliable transportation or public transit.
