We are seeking a dedicated Pharmaceutical Care Specialist to join the Pharmacy team at Laredo Medical Center, a prominent 326-bed acute care facility. This position is vital in delivering comprehensive pharmaceutical care to our patients and collaborating closely with healthcare professionals.
Key Responsibilities:
1. Deliver expert clinical pharmaceutical care to patients.
2. Act as a professional resource for various service areas and the Pharmacy department.
3. Contribute to the development of Therapeutic Equivalence Policies and Programs.
4. Recommend and implement medication protocols and programs.
5. Engage in medication interchanges (IV to PO) as necessary.
6. Participate in Clinical Student programs to foster learning.
7. Conduct thorough Drug Regimen Reviews and address potential Drug Interactions.
8. Provide Drug Information Services to healthcare staff.
9. Support physicians with relevant medication recommendations.
10. Educate patients on their medication regimens and health conditions.
11. Continuously enhance expertise in specialty medicine areas.
12. Monitor parenteral nutrition programs diligently.
13. Facilitate orientation and training for Pharmacy and Hospital staff regarding medications.
Position Requirements:
- Education: Pharm.D. or completion of a residency from an accredited institution.
- Experience: Completion of a PGY1 Residency in a Hospital setting.
- Licensure: Must be licensed to practice in the State of Texas.
- Certifications: A.C.P.E. accredited I.V. Certification and completion of an accredited Residency in Hospital Pharmacy (PGY1/PGY2).
- Additional Skills: BLS certification is required; Board of Pharmacy Specialty certifications are encouraged.
- Preferred: BCOP (Board Certified Oncology Pharmacist) and BCPS (Board Certified Pharmacotherapy Specialist).
