WelbeHealth PACE (Program of All-Inclusive Care for the Elderly) offers a unique opportunity for seniors to maintain their independence within their communities. Under the guidance of the Medical Director, the Nurse Practitioner collaborates closely with WelbeHealth Physicians and the IDT, providing thorough assessments, care planning, and ongoing medical education to our participants.

Key Responsibilities:

  • Conduct in-person comprehensive evaluations, including initial, semi-annual, and unscheduled assessments, along with continuous medical oversight for participants.
  • Partner with physicians and the IDT to formulate individualized care plans for each participant.
  • Assist in discharge planning from both acute and long-term care settings in collaboration with physicians and the IDT.
  • Support the ongoing care of participants residing in nursing facilities through regular visits based on individual needs.
  • Work in conjunction with WelbeHealth physicians, community-based PACE physicians, and the IDT to ensure effective care coordination.
  • At the direction of the Medical Director, analyze outcomes and utilization data, including hospital admissions and specialist referrals, to identify best practices and enhance policies.
  • Participate in a rotating schedule for night and weekend on-call duties.

Qualifications:

  • Graduation from an accredited nursing program.
  • Certification as a Nurse Practitioner with Prescriptive Authority.
  • Current RN State License.
  • A minimum of two years of experience working with the elderly population.
  • Active Drug Enforcement Administration (DEA) registration.
  • Eligibility for enrollment to provide services to Medicare and Medi-Cal participants.
  • Up-to-date knowledge of chronic care and geriatric best practices.
  • Reliable transportation is required.

Salary range for this position is $145,399.15—$174,478.98 annually, plus bonuses and equity. WelbeHealth provides a competitive total rewards package that includes 401k matching, healthcare coverage, and a variety of other benefits. Actual compensation will be adjusted based on experience and qualifications.
