Comprehensive Community Support Specialist

La Clinica de Familia is a leading healthcare provider in Southern New Mexico, dedicated to empowering and enriching families, individuals, and communities through quality medical, dental, behavioral health, and educational services. We are seeking a highly skilled and compassionate Comprehensive Community Support Specialist to join our team.
The ideal candidate will have a strong background in human services and experience working with diverse populations. As a CCSS, you will be responsible for coordinating and providing services and resources to individuals and families, promoting recovery, rehabilitation, and resiliency. Your core competencies will include excellent communication skills, time management, and the ability to work independently and as part of a team.
Key Responsibilities:
- Coordinate services and resources to promote recovery, rehabilitation, and resiliency
- Address goals in areas such as parenting skills, training and behavior management, independent living, learning, socializing, and recreation
- Develop and maintain strong relationships with community partners and stakeholders
- Provide education and support to individuals and families on health and wellness topics
Requirements:
- Bachelor's degree in a human service field from an accredited university and one year of relevant experience
- Associate's degree and two years of experience working with the target population
- High school graduate or GED and three years of experience working with the target population
- Valid NM driver's license and current automobile insurance
- Bilingual (Spanish/English) a plus
