Position Summary

The Speech Language Pathologist is tasked with providing exceptional patient care in accordance with the individual treatment plans established by the patient's physician. Responsibilities may include conducting assessments, implementing prescribed therapies, developing care plans, and collaborating with other professionals in the patient's multi-disciplinary team. This role also encompasses supply management, emergency response, care coordination, health education, and training for family members.

PRN Visits are essential for expanding services in various communities

Key Responsibilities:

Deliver Speech Therapy services as directed by the physician. Adhere to established policies and procedures while complying with all relevant patient care standards, licensure requirements, and state regulations. Create a comprehensive written evaluation outlining Speech Therapy needs, objectives, and anticipated outcomes in alignment with the referring physician's treatment plan. Develop a detailed Plan of Care focusing on Speech Therapy goals and outcomes. Maintain the care plan for assigned patients in accordance with established Speech Therapy objectives and results. Engage in service coordination (including discharge planning) with healthcare teams, physicians, family members, and other stakeholders. Uphold patient and organizational confidentiality. Foster effective communication among staff, healthcare team members, and families. Act as a resource consultant for clinical personnel. Provide physicians and Aveanna with written assessments of patient needs, goals, and progress.

Qualifications:

Master's degree in Speech-Language Pathology from an accredited institution. Valid and unrestricted SLP license to practice in the relevant state. Current CPR certification. Minimum of one (1) year of recent experience as a Speech Language Pathologist in an acute care environment is preferred.
