Key Responsibilities
  • Monitor physician and ancillary visits and orders.
  • Address resident observations and concerns with the appropriate coordinators.
  • Provide guidance and support to nursing staff as necessary.
  • Oversee discharge planning for residents, including arranging home services.
  • Manage certifications and recertifications for residents in skilled nursing.
  • Facilitate weekly meetings regarding Medicare, therapy, and resident care.
  • Act as a liaison for therapy services.
  • Conduct reassessments for residents and hospital visits when needed.
  • Support the restorative care program.
  • Participate in daily stand-up meetings.
  • Assist in managing the survey process.
  • Support physician rounds.
  • Serve as the on-call nurse in charge on a rotating schedule.
  • Perform additional duties as assigned.
Qualifications
  • Education: Graduate of an accredited nursing program.
  • Licensure/Certification: Valid Ohio Registered Nurse (RN) license.
  • Experience: At least three years of nursing experience in long-term care; experience in restorative nursing or as a nursing supervisor is preferred.
  • Computer Skills: Proficient in electronic medical record systems.

  • Detail-oriented with the ability to lead and provide high-quality service in a person-centered environment.
  • Knowledgeable about clinical standards, regulations, and reimbursement related to long-term care and Medicare/Medicaid.
  • Respect the autonomy and dignity of all residents.

  • Empower residents by involving them in decision-making processes.
  • Make timely and accurate decisions regarding resident care and emergencies.
