We are seeking a highly skilled Registered Dietitian to join our team on a per diem basis, including weekends. As a key member of our healthcare team, you will provide optimal nutritional care to assigned patients through appropriate assessments, interventions, education, and medical team communication.
Key Responsibilities- Utilize standards of dietetic practice to provide optimum nutritional care to assigned patients.
- Complete nutritional consults in a timely and efficient manner.
- Assess nutrition needs and determine nutrient requirements and appropriate diets according to patient age, disease-specific needs, and behavioral and metabolic responses to medical and nutritional care.
- Collaborate and communicate with patients and healthcare teams to assess needs, establish mutual goals, and determine the most appropriate care plan to meet goals.
- Identify desired outcomes of nutrition care and verify that each nutrition-related problem listed on the medical record has been addressed.
- Conduct mealtime visits to evaluate patient acceptance, tolerance, or rejection of nutritional regimens.
- Provide patient and family nutrition education/counseling, as needed, and provide appropriate materials to facilitate learning and change.
- Evaluate the effectiveness of nutrition interventions/education in terms of patient therapeutic outcomes and behavioral responses.
- Document nutrition care provided, including communications with medical teams, results, and revisions in nutritional care, patient behavioral responses, and therapeutic outcomes to nutrition interventions and/or education.
- Inform healthcare teams about regulations, professional guidelines, departmental policies, and procedures related to nutrition care.
- Attend and participate actively in weekly Patient Care Rounds and Discharge Planning meetings.
- Stay current with research findings to enhance knowledge and skills and share knowledge with nutrition staff and healthcare teams.
- Communicate the need for and priorities of relief coverage to head nurses for own units or other units in a timely manner.
- Bachelor's Degree in Nutrition or related field, required.
- Registered Dietitian, required.
- Minimum of one (1) year clinical experience in acute or long-term care facilities.
- Good interpersonal and communication skills required.
