We are seeking a dedicated Clinical Coordinator to contribute positively to our outpatient dialysis center. This role offers the opportunity to significantly improve the quality of life for our patients and their families who are navigating the challenges of end-stage renal failure or chronic kidney disease.
Responsibilities:- Coordinate care and services for patients with End-Stage Renal Disease (ESRD), monitoring laboratory values, tracking quality metrics and outcome management through QAPI.
- Ensure that you and your clinical team provide the highest standard of care, prioritizing patient safety, comfort, and wellbeing.
- Flexibility in scheduling is necessary, with the ability to work mornings, weekends, and holidays.
- A current Registered Nurse (RN) license in the respective state is mandatory.
- Current CPR certification is required.
- A minimum of 18 months of registered nursing experience is necessary.
- Dialysis Charge RN readiness approval from the Clinical Nurse Manager or RN-licensed Facility Administrator/General Facility Administrator is required.
- Certification as a Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN) is preferred.
