We are seeking a skilled RN Assisted Living Coordinator to join our team at Southern Hills Hospital and Medical Center. As a key member of our behavioral health team, you will be responsible for coordinating the intake process for patients, ensuring seamless transitions between departments, and providing exceptional patient care.
Responsibilities:
- Coordinate the intake process for patients, including assessing patient needs and developing individualized care plans.
- Collaborate with hospital departments to ensure efficient and effective care delivery.
- Provide education and training to patients and families on behavioral health concepts and treatment options.
- Participate in community events and health fairs to promote behavioral health awareness and education.
- Stay up-to-date on current behavioral health concepts, principles, and methods of treatment.
- Understand and adhere to state and federal standards regarding behavioral health assessment, petition, medical screening, and patient rights.
Requirements:
- RN Diploma or Associates degree in nursing or a related mental health field.
- 1-3 years of experience in a behavioral healthcare setting or clinical treatment setting.
- Licensed as a Registered Nurse in Nevada or a LCSW, LFT, LPC, Mental Health Counselor Intern, LMSW, or Clinical Social Worker.
