Job Title: Case Manager

Community Renewal Team is seeking a skilled and compassionate Case Manager to join our team. As a Case Manager, you will play a vital role in supporting individuals and families in need, providing guidance, and connecting them with community resources.

Key Responsibilities:
  • Conduct assessments of customer needs and develop individualized service plans
  • Coordinate referrals to Community Renewal Team programs and initiatives, as well as to other community resources
  • Provide case management services, including counseling, advocacy, and support
  • Monitor and document customer progress and adherence to individualized program plans
  • Collaborate with community partners to ensure seamless service delivery
Requirements:
  • Associate or bachelor's degree in a human services field or related field
  • Minimum 3-5 years of experience in case management or a related field
  • Active certification in HIV/AIDS, drug and alcohol abuse, and/or counseling
  • Knowledge of human services programs, community resources, and the court system
  • Strong communication, organizational, and interpersonal skills
