We are seeking a skilled Patient Care Coordinator - Home Health to join our team at PeaceHealth. As a key member of our Home Health team, you will play a vital role in facilitating the uninterrupted continuing medical care of our patients.
Key Responsibilities:
- Assist clinical staff with onboarding of patients, including detailed evaluations of patient needs, triage of referrals, and coordination of care.
- Evaluate patient care plans for regulatory compliance, skilled need, and documentation, and assist staff with development of patient-specific care plans.
- Coordinate clinical services and facilitate scheduling of patient care services/visits based on assessed clinical needs of the patient.
- Support clinical education as directed for Home Health/Hospice care, and assist in the development of clinical training tools and resources.
- As part of on-going discharge planning and care coordination, evaluate interventions and outcome of patient care, identify barriers to positive outcomes, and implement actions and education to support realization of care objectives and quality outcomes.
Requirements:
- Graduate from an accredited school of nursing. Bachelor's Degree Preferred: Nursing.
- A minimum of 5 years of clinical experience with at least 2 years of Home Health, hospice, or home infusion experience.
- Licensed as a Registered Nurse in the state of Oregon.
- Basic Life Support certification from the American Heart Association or the American Red Cross.
- Drivers License with proof of insurance.
