About the Role:

We are seeking an experienced Occupational Therapist to join our Home and Community Rehabilitation program in the Sacramento area. As a key member of our team, you will provide functional rehabilitation services to individuals with neurological conditions in their natural home and community environment.

Responsibilities:

  • Perform evaluations and develop treatment plans to promote independence and functional capabilities.
  • Regularly re-assess effectiveness of treatment plans and communicate patient progress to the treatment team and family members.
  • Collaborate with other disciplines to ensure coordination of care and enhance patient outcomes.

Requirements:

  • Minimum of a bachelor's degree in Occupational Therapy from an accredited program.
  • Current, unrestricted license as an Occupational Therapist in the state of California.
  • Current CPR Certification and a minimum of one year's work experience as an Occupational Therapist.
