Job Overview

We are seeking a skilled Registered Nurse Case Manager to join our team at Medical City Plano. In this role, you will play a vital part in delivering exceptional patient-centered care and promoting quality outcomes.

About the Role

The successful candidate will have a current Texas state license as a Registered Nurse and at least 3 years of clinical hospital nursing experience. A background in case management is also essential, with certification in case management being preferred. The ideal candidate will be able to perform comprehensive assessments of patients' psychosocial, medical, and discharge needs, while also coordinating the plan of care and driving the discharge plan in collaboration with the multidisciplinary healthcare team.

Responsibilities:

  • Performing comprehensive assessments of patients' psychosocial, medical, and discharge needs
  • Coordinating the plan of care and driving the discharge plan in collaboration with the multidisciplinary healthcare team
  • Reassessing patients' clinical conditions as indicated and developing strategies to mitigate readmission risk
  • Involving patients and their families in identifying and clarifying needs and expectations to develop mutual and realistic goals
  • Making appropriate referrals to third-party payer, disease, and case management programs for recurring patients and patients with chronic disease states
  • Facilitating patient throughput with an ongoing focus on effective care transitions, quality, and efficiency
  • Documenting professional recommendations, discharge plans, care coordination interventions, and case management activities
