Job Title: Telemedicine Physician Assistant

We are seeking a skilled Physician Assistant to join our Telemedicine team at Mobile Healthcare Partners, Inc. The ideal candidate will have a strong background in family medicine or internal medicine and be able to work effectively in a remote healthcare setting.

Responsibilities:
  • Lead the Acute Management Team for homebound patients that Mobile Primary Care is the Primary Care Provider.
  • Responsible for daily coordination with triage staff to identify and complete acute telemedicine visits and subsequent service coordination.
  • Lead the ER/Hospital Avoidance Team.
  • Responsible for determining when it is appropriate to send a Mobile Primary Care Medical Assistant (MA) or Nurse to the patient home with telemedicine and acute care stabilization materials.
  • Visit the patient in their home to stabilize and coordinate care.
  • Maintain accurate and up-to-date electronic health records for each patient.
  • Review reports on patient outcomes and contribute to quality improvement initiatives.
Qualifications:
  • A minimum of 2 years clinical experience as an NP/PA in Family Medicine or Internal Medicine Environment; 3-5 years is preferred.
  • NP/PA must have a current and unrestricted DEA License (NP/PA will be required to obtain and maintain a Scope of Practice/Collaborative Agreement with a state licensed physician prior to delivery of health care).
  • Current and unrestricted New York state license. Must comply with all state requirements and regulations.
  • Possess Basic Life Support (BLS)/Cardiopulmonary Resuscitation (CPR) certification prior to performing work.
  • Working knowledge of Microsoft Office Software and excellent computer maneuverability.
  • Must have exceptional diplomatic communication skills and experience in handling difficult customer situations.
  • Must be detailed oriented and have the ability to multi-task.
  • Must possess a Master's Degree in Nursing with completion of a university-based program for Nurse Practitioner.
  • Must possess relevant certification from American Nurses Association or another nationally recognized certifying body. Nurse practitioner must be licensed or otherwise recognized as a nurse practitioner in New York State.
