Abode Care Partners is a leading provider of integrated medical services, caring for individuals from post hospitalization to home in various settings. As a compassionate Senior Healthcare Professional, you will provide personalized medical care to seniors and patients with complex needs.
We are seeking an experienced Advanced Practice Registered Nurse (APRN) to join our team. In this role, you will perform evidence-based medical assessments, collaborate with other healthcare professionals, and maintain up-to-date patient records.
Key Responsibilities:
- Plan, direct, and evaluate patient medical needs
- Examine and treat chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses
- Order and interpret laboratory and diagnostic testing as needed
- Collaborate with caregivers and other healthcare professionals in providing interdisciplinary care and maintaining a comprehensive plan of care
- Provide referrals to other healthcare practitioners when necessary
- Maintain accurate and detailed patient medical records
Requirements:
To be successful in this role, you will need:
- A valid state APRN license
- Current National Board Certification
- A Master's degree in Nursing (MSN)
- At least 5 years of nursing experience; at least 2 years of APRN experience preferred
- Excellent interpersonal communication skills – verbal and written
- Strong time management skills with accountability for daily workflow
- Proficiency in medical instruments and equipment
- Knowledge of HIPAA and OSHA regulations
- Demonstrated proficiency in computer-based data management programs, electronic medical record (EMR) systems, and Microsoft Office applications
- A valid driver's license with automobile insurance
