Key Responsibilities- Utilization Management: Ensure medical necessity and prevent denials by coordinating care and services.
- Transition Management: Promote timely throughput, safe discharge, and prevent avoidable readmissions by assessing patient needs and coordinating care.
- Care Coordination: Demonstrate throughput efficiency while ensuring care is provided at the right sequence and level.
- Compliance: Ensure adherence to state and federal regulatory requirements, TJC accreditation standards, and Tenet policy.
- Education: Graduate of an accredited School of Nursing, BSN preferred.
- Certifications: Current RN licensure in the State of Massachusetts, 2 years RN acute care hospital experience, and ACM certification preferred.
