About the Role

We are seeking a Licensed Practical Nurse (LPN/LVN) to join our team at Aveanna Healthcare, one of the nation's largest homecare providers.

Job Summary:

The successful candidate will be responsible for providing skilled nursing care to patients in their homes, under the supervision of a Registered Nurse. This role requires a high level of clinical expertise, excellent communication skills, and the ability to work independently.

Key Responsibilities
  • Deliver Quality Patient Care: Provide and document skilled nursing care to patients, following physician orders and developed care plans.
  • Clinical Assessment and Documentation: Continuously observe and assess patient condition and care needs, reporting changes to supervisors and physicians as necessary.
  • Nursing Actions and Coordination: Participate in developing and updating nursing care plans, take appropriate actions based on assessments, and achieve expected outcomes.
  • Infection Control and Education: Implement infection control measures to protect staff and patients, educate patients and families regarding disease processes and self-care techniques.
Requirements
  • Certifications and Qualifications: Current, unrestricted state license as a Licensed Nurse in the state of practice, current CPR certification, and demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care policies.
  • Experience: Six (6) months of recent experience as a Licensed Nurse in a clinical care setting, preferably in home health or pediatrics.
