  • State ID or Driver's License
  • Social Security Card

  • New York License and Registration (RN, LPN, CNA)
  • Two Reference Letters
  • Physical Exam with PPD, MMRV, and Hep B lab results within the last 10 months
  • Drug Screening (10 Panel - within the last 10 months)
  • Flu Vaccination
  • High School Diploma or Equivalent (RN, LPN, CNA)
  • CPR Certification (RN, LPN & optional for CNA)
  • Malpractice Insurance (RN, LPN & optional for CNA)
