Position Overview: The Practical Nursing Specialist provides exceptional nursing care by applying the nursing process effectively. Collaborates with healthcare team members to uphold professional nursing standards. Executes responsibilities in alignment with organizational policies, mission, and regulatory guidelines. Directs and coordinates care activities to promote a patient and family-centered approach.

Key Responsibilities:

  1. Accountable for comprehensive nursing care of residents as directed by the Medical Director, attending physician, and Director of Nursing.
  2. Conducts daily visits to residents, assessing their physical, emotional, and social needs, and completes necessary documentation.
  3. Communicates resident status to clinical coordinators and physicians through written and verbal updates.
  4. Engages in the resident care review process, contributing to goal setting, problem-solving, and necessary adjustments through a multidisciplinary approach.
  5. Prepares assessments and care plans, ensuring accurate documentation in medical records according to established protocols.
  6. Responsible for the precise administration of medications and maintaining accurate records in the Medical Administration Record (MAR).
  7. Supervises and delegates tasks to direct care staff based on resident needs, ensuring completion of daily responsibilities.
  8. Manages nursing supply inventory by monitoring levels, anticipating needs, placing orders, and verifying receipt of supplies.

Qualifications:

  • Valid LPN licensure from the State of Michigan
  • BLS Certification
  • Physical ability to frequently lift, push, and/or pull objects up to 35 pounds

Preferred Experience:

  • Previous experience in geriatric care
