We are seeking a dedicated Skilled Nursing Care Provider to join our team at SSM Health. This role is a great opportunity for individuals who are passionate about delivering high-quality patient care and making a positive impact in the lives of others.

Job Summary:

As a Skilled Nursing Care Provider, you will be responsible for providing skilled nursing care to residents in long-term care settings. You will work closely with the healthcare team to develop and implement individualized care plans that meet the unique needs of each resident.

Responsibilities:

  • Provide quality care to acute, subacute, and chronically ill residents
  • Administer medications and treatments according to established standards of care
  • Maintain accurate inventory of medications and ensure timely distribution to residents
  • Monitor residents' conditions and report significant changes to physicians as needed
  • Collaborate with auxiliary staff to ensure seamless delivery of care
  • Communicate effectively with residents, families, and healthcare team members to ensure comprehensive care

Requirements:

  • Graduate of an accredited school of nursing
  • No experience required, but previous experience in long-term care or skilled nursing is preferred
