Centre HomeCare, Inc. is seeking a licensed Speech Language Pathologist to join our delivery care team. As a key member of our team, you will assess, diagnose, and treat patients with communication and swallowing disorders in their place of residence.

Our ideal candidate will have experience working with cognitive-communication and swallowing disorders, which may occur after a stroke, traumatic brain injury, illness, surgery, injury, or dementia. You will work closely with our delivery care team to promote a continuum of care and ensure positive patient outcomes.

Responsibilities:
  • Conduct initial assessments and develop individualized treatment plans
  • Coordinate with the delivery care team to promote a continuum of care
  • Record notes and information in our health information system
