Job Summary

PACE Southeast Michigan is seeking a skilled and compassionate Nursing Assistant - Participant Care Associate to join our team. As a key member of our care team, you will provide supportive services to our participants, promoting their physical and emotional well-being in their homes, communities, and day health centers.

Key Responsibilities
  • Develop and implement individualized care plans in collaboration with our registered nurses and other healthcare professionals.
  • Provide personal care, assistance with activities, meal preparation, and transportation to appointments.
  • Assist with medication reminders, laundry services, and homemaking tasks to ensure our participants' comfort and independence.
  • Document accurate and timely information about each participant's care and services provided.
  • Communicate effectively with participants, families, and healthcare team members to ensure seamless care coordination.
Requirements
  • High school diploma or equivalent required.
  • Valid Michigan driver's license and reliable transportation.
  • CPR certification and one year of experience working with frail or elderly populations.
  • Optional certification in direct care, nurse assistant, medical assistant, or patient care technician.
  • Ability to work independently, prioritize tasks, and maintain a flexible schedule.
  • Excellent communication and interpersonal skills, with a focus on empathy and respect for participants' physical, spiritual, and emotional well-being.
Working Conditions

Our participants' homes and communities present variable working conditions, with potential exposure to communicable diseases and chemicals. You will be supervised monthly by our registered nurses and participate in regular in-service training to maintain your skills and knowledge.
