Job Summary

BAYADA Home Health Care is seeking a compassionate and dedicated Admissions Registered Nurse (RN) to join our team in Central New York. As a key member of our home health care team, you will be responsible for conducting Start of Care (SOC) Admissions, following designated care plans, and making home visits to clients in designated geographic territories.

Responsibilities
  • Conduct Start of Care (SOC) Admissions - Utilize OASIS to assess patient needs and develop individualized care plans.
  • Follow Designated Care Plans - Implement care plans in accordance with patient needs and preferences.
  • Make Home Visits - Provide high-quality patient care in the comfort of their own homes.
  • Perform Assigned Duties - Administer medication, provide wound care, and perform other treatments as needed.
  • Monitor Client Conditions - Report changes to Clinical or Client Services Manager.
  • Follow Up with Doctors' Orders - Execute and properly document doctors' orders.
  • Perform Client Assessments - Conduct regular assessments to ensure patient needs are being met.
  • Accurately Document Observations - Utilize a state-of-the-art touch pad tablet to document patient care and services provided.
Qualifications
  • Current License as a Registered Nurse in New York State
  • Minimum of Two Years of Recent, Verifiable Clinical (Medical/Surgical) Experience
  • Prior Home Care and OASIS Experience Required
  • Graduation from an Accredited and Approved Nursing Program
  • Ability to Work Independently and Manage Time Effectively
  • Strong Interpersonal Skills
  • Solid Computer Skills, Prior Experience with Electronic Medical Records (EMR) Preferred
  • Ability to Travel to Cases as Assigned
