Full Time Healthcare Assistant - Aging Population

At Visiting Nurse Service of New York, we are committed to delivering high-quality healthcare services to our patients. As a Full Time Healthcare Assistant, you will play a crucial role in ensuring that our residents receive the best possible care. Your primary responsibility will be to collaborate with our site staff to ensure that residents engage in self-care management health strategies. You will also provide health-focused outreach to seniors who may benefit from our supportive services. Your duties will include implementing health promotion activities, such as health screening, health assessments, health care linkages, health care referrals, and health presentations. You will also make telephone calls and home visits to individual residents to assess their needs and support successful community living activities. Additionally, you may make referrals to appropriate health services based on the assessed needs of the resident. You will also provide health care advocacy by acting as a liaison between various healthcare systems during times of transition. Furthermore, you will participate in the Quality Assurance Program and Education Program as requested by the Clinical Manager. You will also refer residents needing direct patient care to their physician or to appropriate healthcare services to address their particular needs. Finally, you will document all nursing interventions provided as required by site location.

Qualifications

Licenses and Certifications:
License and current registration to practice as a Registered Professional Nurse in New York State is required. CPR/BLS certification is also required.

Education:
An Associate's Degree in nursing is required, and a Bachelor's Degree in nursing is preferred.

Work Experience:
A minimum of two years of experience working as an RN in LHCSA or CHHA is required, and prior experience working with an aging population and providing clinical assessments of their health needs is also required.

Compensation:
$77,200.00 - $96,500.00 Annual.
