Position Responsibilities:

  1. Ensures that all care provided aligns with the scope and established standards of safe professional nursing practice.
  2. Collaborates with multidisciplinary team members (MD, NP, nurses, social worker, therapists, dietician) to meet patient clinical needs and monitor care outcomes.
  3. Adheres to best practice recommendations and implements evidence-based interventions for optimal patient outcomes.
  4. Practices within a primary nursing model, maintaining accountability for a designated group of patients under RN oversight.
  5. Participates in care planning activities, supporting autonomy and individual choice whenever feasible.

  7. Documents relevant information regarding care delivered in the designated areas of the patient's record using electronic medical records.

  9. Identifies and seeks necessary resources to ensure safe and effective care delivery.
  10. Delegates and supervises clinical care provided by nursing assistants.
  11. Takes responsibility for personal learning needs and professional development.

  22. Acts as a coach and mentor to patient care associates.
  23. May assume charge nurse responsibilities under RN oversight, which may include:
  • Providing shift reports to incoming staff.
  • Offering clinical and supervisory oversight during shifts.
  • Delegating assignments to the team for the shift.
  • Auditing and ensuring completion of admissions or discharges during shifts with RN co-signature.
  • Attending rounds (wound, physician, etc.).
  • If acting as charge nurse, may develop patient care assignments and contribute to performance evaluations and staff education on policies and procedures.

Preferred Qualifications:

  • Graduate of an accredited nursing program required.
  • Current Massachusetts LPN license in good standing.
  • Previous experience preferred.
  • Understanding of and support for the care model at Hebrew SeniorLife required.
  • Must be a professional, proactive, collaborative, conscientious, and results-oriented individual with a positive demeanor.
  • Excellent oral and written communication skills, strong intuition, and adaptability to changing priorities.
  • Strong organizational and analytical skills.
  • Ability to innovate, think strategically, manage multiple projects, and handle challenging situations.
  • Motivated to learn and flexible to change.
  • Computer literacy required.
  • Excellent English language skills, both verbal and written.
