Job Title: Haitian Community Health Navigator

Join the YMCA of the Roses team as a Haitian Community Health Navigator, a vital role in supporting the health and well-being of the Haitian community. As a member of the New American Welcome Center (NAWC) team, you will provide navigation to healthcare services, connections to community resources, healthcare education, appointment follow-ups, and support to Haitian community members in their healthcare journey.

About the Role:

The Haitian Community Health Navigator will help patients navigate the complex healthcare system and act as a liaison and advocate for the Haitian population, with a focus on obstetric and pediatric patients receiving care at WellSpan practices in the York County community.

Key Responsibilities:
  1. Connect patients with appropriate care channels, including pharmacy, insurance, billing, and referral, and provide them with the information they need to successfully navigate their care journeys.
  2. Identify barriers to care and refer patients as needed to additional resources, leveraging HereToHelpAll, and follow standard work for the Navigator program.
  3. Conduct follow-up with patients in between episodes of care to promote a seamless and predictable healthcare experience, track patients, and conduct intensive outreach to engage them in care.
  4. Help patients attend appointments, assist with transportation coordination, and facilitate and schedule patient appointments and referrals as needed.
  5. Track navigation services, record messages, and contribute to healthcare tracking workflows, collect patient and community member information, and relay it to clinical and care management team members.
  6. Educate patients on care processes, respond to patient inquiries, and assist patients with the completion of paperwork.
  7. Establish relationships with Federally Qualified Health Centers (FQHCs), low-cost and free clinics, and other agencies, and serve as a link between the person and community resources, identifying a medical home where the patient has access to consistent healthcare.
  8. Refer patients to or assist patients with health education resources for increased knowledge to better self-manage their health and linkage to resources for improved quality of life.
  9. Support reporting requirements for tracking of metrics related to the Community Health Navigator activities.
Qualifications:
  • Fluency, written and spoken, in both Haitian Creole and English required.
  • Understanding of the Haitian culture and history.
  • Familiarity with immigrant, refugee, and newcomer needs and experiences.
  • High school diploma required; college degree preferred.
  • Experience in healthcare or care coordination preferred.
  • Effective communication skills, both written and verbal, needed to communicate with care teams.
  • Ability to interact with newcomers to the community from Haiti.
  • Ability to use computer programs to support activities and learn new platforms.
  • Ability to establish and maintain collaborations with community organizations.
  • Ability to relate effectively to diverse groups of people from all social and economic segments of the community.
  • Awareness of social justice, faith, and domestic and international issues needed.
  • Ability to meet WellSpan Health compliance program requirements that support WellSpan's values and standards for professionalism, integrity, and ethics.
  • Ability to comply with WellSpan Health privacy program to safeguard patient information and business and operational information of the organization.
Work Environment and Physical Demands:

Able to stand for extended periods, lift 30lbs, bend, reach, squat, and sit for long periods. Ability to plan, lead, and participate in activities.
