Job Summary:

We are offering a competitive salary of $85,000 - $110,000 per year, depending on experience, for this full-time position. The successful candidate will be responsible for promoting optimal management of clinical resources, conducting timely initial admission assessments, and coordinating the discharge needs of patients with necessary internal and external providers.

Responsibilities:
  • Conduct timely initial admission assessments to ensure quality and appropriateness of patient care
  • Coordinate the discharge needs of patients with necessary internal and external providers
  • Implement, reassess, and evaluate patient condition daily, ensuring updated plan of care is documented in the EMR
  • Promote efficiencies in the care delivery system and reduce barriers to effective service
  • Maintain compliance with all company policies, procedures, and standards of conduct
Requirements:
  • Licensed or eligible for license in Kentucky
  • Basic Life Support (BLS) accredited by the American Heart Association (AHA)
  • Bachelor's degree from an accredited college or University, or RN with a Bachelors Degree in Business or Health Care Administration preferred
  • 5+ years of care management experience in an acute hospital setting
Preferred Qualifications:
  • Certification in a nursing specialty area or CCM
