LPN Medication Care Manager

We are seeking a highly skilled LPN Medication Care Manager to join our team at Sunrise Senior Living. As a Medication Care Professional, you will be responsible for providing the highest degree of quality care and services by administering medication and treatments in a safe and organized manner.
Responsibilities- Medication Administration: Provides the highest degree of quality care and services by administering medication and treatments in a safe and organized manner.
- Reviews and Documents: Reviews, reads, notates, and initializes Daily Log to document and learn about pertinent information about residents.
- Medication Updates: Receives medication updates from Resident Care Director (RCD) or Wellness Nurse.
- Medication Pass: Administers, assists with, and observes medications and treatments for each resident using the medication administration record and the Six Rights of Medication Pass.
- Confidentiality: Maintains confidentiality of all resident information including resident medication among other residents.
- Reporting: Reports all resident concerns made while administering the medication to the RCD or Wellness Nurse.
- Medication Cart: Restocks medication cart after all medication passes.
- Training and Development: Participates in regular training by RCD and neighborhood coordinators.
- LPN License: LPN, LVN, or state/province specific licensed nurse credential.
- Certifications: Maintains certifications and ongoing training and re-education as required by Sunrise and state/provincial regulations.
- Communication Skills: Possesses written and verbal skills for effective communication and level of understanding.
- Problem Solving: Demonstrates good judgment, problem solving and decision-making skills.
