Early Intervention Physical Therapist

We are seeking a highly skilled and compassionate Physical Therapist to join our team at Criterion Child Enrichment. As an Early Intervention Physical Therapist, you will play a vital role in providing essential services to families with children birth to 3 years of age with developmental delays or at risk for developmental delays.
Job Summary:The successful candidate will be responsible for providing home-based services to families across the catchment area, collaborating with community providers to ensure continuity of services, and participating in screenings, assessments, child groups, parent groups, center visits, and home visits as a member of a trans-disciplinary team.
Key Responsibilities:- Provide home-based services to families across the catchment area, utilizing a parent coaching model in work with families.
- Collaborate with community providers to ensure continuity of services for families, including child care providers, medical providers, social service agencies, and specialty providers serving children with autism or hearing/vision loss.
- Participate in screenings, assessments, child groups, parent groups, center visits, and home visits as a member of a trans-disciplinary team.
- Provide service coordination to assigned caseload, including coordinating the development, monitoring, and review of the Individual Family Service Plan.
- Current licensure as a Physical Therapist by the Massachusetts Board of Registration in Allied Professional Health.
- Two (2) years of pediatric experience preferred.
- Effective verbal and written communication skills.
- Working knowledge of Word, Excel, Adobe, and Office365, including Outlook, OneDrive, and Teams.
- Ability to use computer-based technology, including a computer/laptop and cell phone.
- Valid driver's license and reliable transportation required.
