Hospice Clinical Liaison Specialist

We are seeking a highly skilled and compassionate Hospice Clinical Liaison to join our team at FC Compassus LLC. As a key member of our organization, you will play a vital role in promoting the Compassus philosophy and values, while providing exceptional care to our patients and their families.
Key Responsibilities- Model the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation in all interactions with patients, families, and healthcare professionals.
- Assist referral sources in identifying patients with care needs that match the services provided by hospice.
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
- Collaborate with community, hospital, and facility education representatives to meet growth and development targets and maintain market acceptance and allegiance.
- Facilitate patient discharge from hospital to home by establishing initial needs and coordinating transportation, DME, medication needs, supplies, and admission by a home hospice nurse.
- Perform other duties as assigned, including attending staff meetings, educational programs, and community events.
- Active and unencumbered Registered Nurse license, or Licensed Practical/Vocational Nurse, in state(s) of employment required.
- Two (2) to three (3) years of related experience required, with hospital and/or long-term care clinical experience highly preferred.
- Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
- Strong written and verbal communications, with ability to effectively present information and respond to questions from leaders, team members, investors, and external parties.
The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds.
Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected.
