The Community Health Worker plays a vital role in providing outreach and engagement with eligible Managed Care Plan members and supporting care management through transportation assistance to assigned appointments and social services.

Key Responsibilities:

  • Demonstrate the Telecare mission, purpose, values, and beliefs in everyday language and contact with internal and external stakeholders
  • Provides daily support to Lead Care Coordinator related to care coordination for members
  • Engages and conducts in-person outreach with eligible Managed Care Plan (MCP) Members
  • Accompanies MCP Member to office visits, as needed, and in the most easily accessible setting, within MCP guidelines
  • Distributes County provided health promotion materials
  • Extends County provided health promotion and self-management training
  • Arranges or provides transportation when needed and appropriate
  • Provides assistance with linking member to social supports
  • Contacts MCP Member to facilitate visit with care coordinator
  • Advocates on behalf of MCP Members with healthcare professionals
  • Uses motivational interviewing, trauma-informed care, and harm-reduction practices
  • Monitors treatment adherence (including medication)
  • Required qualifications include High School Diploma or GED, two years' experience providing mental health or intensive case management services, and Paraprofessional with or without lived mental health experience
