Job Title: Inpatient Pharmacist

We are seeking a highly skilled Inpatient Pharmacist to join our team at the County of Santa Clara CA. As a key member of our pharmacy team, you will play a vital role in ensuring the safe and effective use of medications for our patients.

Key Responsibilities:
  • Medication Management: Directly responsible for the selection, preparation, and distribution of medications, chemicals, solutions, and materials for medical and scientific use.
  • Team Leadership: Functionally direct supportive personnel involved in medication management activities.
  • Clinical Judgments: Provide clinical judgments necessary to proper utilization and administration of medications in cooperation with medical and nursing staff.
  • Patient Counseling: Provide patient counseling and medication education services.
Key Requirements:
  • Education: Possession of an academic degree in Pharmacy from an accredited College of Pharmacy.
  • Licensure: A valid license to practice as a pharmacist in the State of California.
  • Knowledge and Abilities: Sufficient education, training, and experience to perform the above named typical tasks and to demonstrate possession of the following knowledge and abilities:
    • Pharmacology: Knowledge of prescription and non-prescription drugs and drug products and their indications, efficacy, dosage, mechanisms of action, fate and disposition, major contraindications, and potential side effects.
    • Pharmacokinetics: Knowledge of the influence of patient's age, sex, concomitant disease states, concurrent drug therapy, foods, and diagnostic procedures on drug activity and disposition.
    • Pharmacy Practice: Knowledge of physiology, anatomy, pathology, chemistry, and mathematics as they relate to pharmacy practice.
    • Drug Information: Knowledge of major reference sources to answer drug information/ingestion questions.
    • Pharmaceutical Manufacturing: Knowledge of pharmaceutical manufacturing methods.
    • Legislation: Knowledge of legislation governing pharmacy practice and hospital pharmacy services/hospital operations as they relate to pharmacy services.
  • Interpersonal Skills: Ability to establish and maintain cooperative working relationships and provide functional direction to para-professional personnel.
  • Counseling Skills: Ability to effectively counsel patients regarding use, possible side effects, and storage of medications.
  • Communication Skills: Ability to effectively advise and consult with medical, nursing, and other personnel regarding properties of drug substances.
  • Problem-Solving Skills: Ability to use and evaluate information in appropriate reference sources and clinical literature.
