Accentra Home Healthcare is seeking a skilled Home Health RN to join our team on a PRN basis. As a Home Health RN, you will provide high-quality patient care in the comfort of their own homes, working closely with our interdisciplinary team to ensure optimal outcomes.
Responsibilities:- Provide professional nursing care to patients in their homes, as prescribed by physicians and in compliance with state and federal regulations.
- Conduct comprehensive assessments, develop and implement individualized plans of care, and coordinate with other healthcare professionals to ensure seamless care transitions.
- Supervise home health aides, educate patients and caregivers on disease management, and provide emotional support to patients and their families.
- Collaborate with the interdisciplinary team to identify and address patient needs, and participate in quality assurance activities to ensure high-quality care.
- Current certification as a Registered Nurse in the state of Oklahoma.
- Previous home health experience preferred, but not required.
- Valid driver's license, reliable vehicle, and valid auto liability insurance.
- Ability to work independently, with minimal supervision, and to prioritize tasks effectively.
