Cancer Care Navigator Job Description

We are seeking a highly skilled and compassionate Nurse Navigator to join our team at UNC Health Care. As a key member of our interdisciplinary team, you will play a vital role in guiding patients through their cancer care journey.

Key Responsibilities:

  • Provide comprehensive barrier assessments in four domains: health promotion, logistics, financial, and emotional and social support.
  • Navigate patients, families, and caregivers through the cancer care continuum, providing resources and interventions as needed.
  • Oversee daily navigation assignments, interventions, data collection, and documentation for the Oncology Patient Navigators.
  • Partner with the Lead Navigator in the onboarding, orientation, and continuing education of the Patient Navigators.
  • Collaborate in quality and research efforts for the Cancer Care Navigation Program.

Requirements:

  • Bachelor's degree in Nursing (BSN) from a state-accredited school of professional nursing.
  • Licensed to practice as a Registered Nurse in the state of North Carolina.
  • Oncology Nursing Certification (ONC) within two (2) years of employment.
