LPN in Assisted Living Job Summary

At Country Meadows, we are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team in an Assisted Living setting. As an LPN, you will have the opportunity to make a meaningful difference in the lives of our residents and their families.

Key Responsibilities:
  • Form strong bonds with residents, their loved ones, and physicians to create personalized care plans.
  • Coach and support co-workers to ensure seamless, quality care is delivered around the clock.
  • Monitor and oversee medications, and educate residents and their families about health and wellness issues.
  • Serve as and share the responsibilities of the Manager on Duty.

Requirements:
  • LPN license in the state where employed.
  • CPR certification.
  • Minimum of one-year experience in assisted living or other healthcare setting.
  • Leadership skills and ability to form relationships with residents, their loved ones, and co-workers.
