Job Summary

We are seeking a highly skilled and compassionate Patient Care Assistant to join our team at Phoenixville Hospital, a member of Tower Health. As a Patient Care Assistant, you will play a vital role in providing exceptional patient care and support to our patients and their families.

Responsibilities
  • Provide nursing care to patients under the supervision and direction of a registered nurse, ensuring a safe and comfortable environment for our patients.
  • Assist with daily living activities, such as bathing, dressing, and toileting, as well as light housekeeping and maintaining cleanliness in patient rooms and common areas.
  • Participate in clinical patient care activities that do not require a nursing license, such as taking vital signs and assisting with medication administration.
  • Respond to call lights, answer phones, and orient patient families and visitors, ensuring a positive experience for all.
  • Maintain accurate and up-to-date electronic medical records, documenting all patient interactions and care activities.
  • Collaborate with the assigned registered nurse to review and discuss patient care plans, ensuring a comprehensive and coordinated approach to patient care.
  • Contribute to efficient patient flow by being aware of room availability and patient intakes and discharges.
  • Clean patient rooms after discharge, properly disposing of all items used during a patient stay and processing reusable items for sterilization and reuse.
  • Demonstrate knowledge and execution of correct patient safety practices, ensuring a safe environment for all patients and staff.
Qualifications
  • High School Diploma or equivalent, or relevant experience may be considered in lieu of education.
  • No prior experience required, but a willingness to learn and grow with our team is essential.
  • BLS Certification is required, and PA Nurse Aide Certification is preferred for the Transitional Sub-Acute Unit (TSU) at Reading Health Rehabilitation Hospital (RHRH).
