WHAT WILL I BE DOING?

CHI is seeking a full-time Lead Case Management Specialist for our Middle Country location. In this role, you will oversee all supervisory functions related to social work services within an emergency housing context. Reporting to the Program Manager, you will collaborate with an interdisciplinary team and supervise the overall operations of Case Managers at your site. Your responsibilities will include:

  • Providing crisis intervention as necessary.
  • Supporting direct case management tasks as required.
  • Conducting regular supervision for Case Managers.
  • Holding one-on-one meetings with team members to review performance goals, achievements, and feedback regularly.
  • Ensuring that quality housing services are delivered effectively and in compliance with established guidelines.
  • Promoting sound social work practices through supervision, including case consultations and performance evaluations.
  • Overseeing the timely submission of all required reports and documentation.
  • Acting as a liaison with agencies responsible for client referrals.
  • Ensuring proper use of company resources.
  • Serving as a role model for Case Management staff in alignment with agency policies.
  • Providing ongoing training and development opportunities for staff.
  • Participating in the hiring process and conducting employee evaluations.
  • Managing coverage and payroll procedures within the department.

WHAT DO I NEED?

  • Education: A BSW or MSW from an accredited institution is required.
  • Experience: 2-5 years of experience in management services is preferred.
  • Certificates or Licenses: A valid driver's license is necessary.
  • Communication: Excellent verbal and written communication skills are essential. Proficiency in English is required; bilingual (Spanish) is a plus.
  • Computer Skills: Basic proficiency in MS Windows, MS Office, and internet usage is required. Familiarity with Foothold Awards System is advantageous.
  • Math Skills: Basic math skills are necessary.
  • Physical Performance: Ability to tour properties and perform physical tasks as needed.
  • Reasoning Ability: Strong decision-making and prioritization skills are essential.
  • Other Skills: Ability to work independently or collaboratively, stay organized, manage responsibilities, work under pressure, meet deadlines, multitask, and maintain attention to detail.

  • Salary: $62,400 annually. Exempt position.
  • Open shifts: Sunday-Thursday, 11 am - 7 pm.
