Job Summary

We are seeking a skilled Licensed Practical Nurse to join our team in a Long Term Care setting. As a LPN, you will be responsible for providing high-quality patient care, working collaboratively with the interdisciplinary team to develop and implement individualized care plans, and ensuring the needs of our residents are met.

Responsibilities
  • Provide direct patient care, including administering medications, performing vital signs, and documenting patient information
  • Develop and implement individualized care plans, taking into account the unique needs and goals of each patient
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
Requirements

To be successful in this role, you will need:

  • A current LPN license in the state of Maryland
  • Basic computer skills and proficiency in electronic medical records
  • Ability to work effectively in a fast-paced environment, prioritizing tasks and managing multiple responsibilities
  • Excellent communication and interpersonal skills, with the ability to work collaboratively with patients, families, and healthcare teams
