Job Summary

As a Licensed Practical Nurse with LifeBridge Health, you will play a vital role in providing high-quality care to our patients in a subacute rehabilitation setting. Under the supervision of the Registered Nurse/Director of Nursing, you will develop and implement individualized nursing care plans to meet the unique needs of our residents.

Responsibilities
  • Provide direct patient care, including administering medications, performing vital signs, and assisting with daily activities
  • Conduct basic physical assessments and document patient information
  • Collaborate with the interdisciplinary team to develop and implement care plans
  • Assume leadership roles as needed to ensure smooth unit operations
Requirements

To be successful in this role, you will need:

  • A diploma in nursing from an accredited school
  • Less than one year of experience in a long-term care or subacute rehabilitation setting
  • A valid license for practical nursing and AHA CPR certification
  • Basic computer skills and proficiency in Synquest, IV Pump, Phillips Monitoring System, Accucheck, and EKG Machine
