This role involves overseeing processes and personnel to ensure effective coordination of care throughout the healthcare continuum, with a particular emphasis on key patient demographics. The goal is to enhance healthcare delivery, elevate patient satisfaction, and minimize costs.
Qualifications:
- Education:
Bachelor's Degree in Nursing (Required)
Master's Degree in Nursing or Healthcare (Preferred) - Experience:
Minimum of 3 years in case management, care transitions, ambulatory care, skilled nursing, or home care (Required)
At least 2 years in a managerial or supervisory role (Preferred) - Licenses/Certifications:
Active RN License in Pennsylvania (Required)
