Position: Community Support Specialist
Schedule: 24 hours per week, including weekends and evenings.
About the Role: In our ACCS Intensive Behavioral Residential Program, Community Support Specialists deliver essential rehabilitation services to individuals transitioning from state facilities. These individuals often require close supervision and tailored treatment due to mental health challenges and complex behavioral needs. Our community-based Group Living Environments (GLE) aim to enhance the quality of life and support the recovery journey of adults facing mental health issues.
Key Responsibilities:
- Assist individuals with Activities of Daily Living (ADLs) such as cooking, cleaning, and financial management.
- Facilitate the development of social skills and self-care routines, including hygiene, budgeting, and community engagement.
- Contribute to the creation and execution of Program Specific Treatment Plans (PSTPs) and participate in regular reviews.
- Ensure compliance with medication administration protocols and maintain accurate documentation.
- Provide transportation for individuals to various appointments and activities, adhering to safety policies.
- Encourage participation in leisure activities and promote independence in choosing these activities.
- Design and implement both group and individual activities that foster community integration.
- Act as a case manager for assigned clients, addressing medical and legal needs as necessary.
- Advocate for clients and model positive behaviors within a supportive environment.
- Maintain open communication with team members and document all relevant data accurately.
Qualifications:
- High school diploma or equivalent; higher education preferred.
- Valid driver's license and a clean driving record.
Experience:
- Prior experience working with individuals with developmental or physical disabilities is preferred.
- Experience in mental health settings is highly desirable.
Skills:
- Proficient in data entry and documentation using electronic systems.
- Ability to understand and apply medication administration guidelines.
- Strong communication skills in English, both written and verbal.
- Physical capability to assist individuals as needed.
