Physical Therapist

We are seeking a skilled Physical Therapist to join our team at Ascend Home Health. As a Physical Therapist, you will be responsible for providing high-quality care to our patients in the comfort of their own homes.
Key Responsibilities:
- Assess patients' physical abilities and develop personalized treatment plans to improve their mobility and function.
- Provide one-on-one therapy sessions to patients, using a variety of techniques and modalities to promote healing and recovery.
- Collaborate with other healthcare professionals to ensure comprehensive care and coordinate patient transitions.
- Document patient progress and maintain accurate records, adhering to agency policies and procedures.
- Stay up-to-date with industry developments and best practices, participating in ongoing education and training to enhance clinical expertise.
Requirements:
- Master's Degree in Physical Therapy from an accredited program.
- Current Physical Therapist Certification in the state of Operations.
- 3+ years of experience in Home Health, Acute Care setting, or Rehabilitation Facility.
