Job Summary

CVS Health is seeking a highly motivated and detail-oriented Case Management Coordinator to join our team. As a Case Management Coordinator, you will be responsible for assessing, planning, implementing, and coordinating case management activities with members to evaluate their medical needs and facilitate their overall wellness.

Key Responsibilities
  • Telephonically and/or face-to-face assess, plan, implement, and coordinate case management activities with members
  • Evaluate the medical needs of members to facilitate their overall wellness
  • Effectively manage a caseload that includes supportive and medically complex members
  • Determine appropriate services and supports due to members' health needs
  • Coordinate with PCP and skilled providers, condition management information, medication review, community resources, and supports
Requirements
  • Bilingual, English, and Spanish
  • Willing and able to travel 50-75% of the time to meet members face-to-face in Miami Dade county and surrounding areas in FL
  • 1+ year of experience in behavioral health or a long-term care setting
Preferred Qualifications
  • Ability to multitask, prioritize, and effectively adapt to a fast-paced changing environment
  • Effective communication skills, both verbal and written
  • Managed Care experience
  • Computer proficiency in Microsoft Word, Excel, and Outlook
Education

Bachelor's degree in social work or related field required

Compensation

The typical pay range for this role is: $21.10 - $36.78

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography, and other relevant factors.

This position is eligible for a CVS Health bonus, commission, or short-term incentive program in addition to the base pay range listed above.
